Two labs can test the same blood and print different normal ranges beside the result. The disagreement is not a mistake, and understanding it is the key to reading your own report.

Vitamin D Normal Range: How to Read Your Blood Test Report

A vitamin D report usually shows a single number and a printed band telling you whether it is deficient, insufficient or sufficient. Most people read the band and stop there. The number deserves more care, because the bands themselves come from guidelines that do not agree, and because the right response depends on who you are and why the test was done.

What does a vitamin D blood test measure?

It measures 25-hydroxyvitamin D, often written 25(OH)D, in the serum. This is the storage form the liver makes from vitamin D obtained through sunlight, food and supplements, and it reflects supply over the previous few weeks. It is not the active hormone. The active form, 1,25-dihydroxyvitamin D, is made mainly in the kidneys and is tightly regulated, so it can look normal or even high while stores are low. That is why 25(OH)D is the standard test for vitamin D status.

What is the normal range for vitamin D?

Many Indian lab reports treat 30 to 100 ng/mL as sufficient, 20 to 29 ng/mL as insufficient and below 20 ng/mL as deficient. These bands follow the Endocrine Society's 2011 guideline, which was written for people at risk of deficiency. Other expert bodies draw the line lower. There is no single normal range that every expert accepts, and that is the most useful thing to know before you read your own number.

These follow the Endocrine Society's 2011 guideline. Multiply by 2.5 for nmol/L, so 20 becomes 50 and 30 becomes 75.

How do you convert ng/mL to nmol/L?

Multiply the ng/mL value by 2.5 to get nmol/L, and divide nmol/L by 2.5 to go the other way. So 20 ng/mL is 50 nmol/L, 30 ng/mL is 75 nmol/L, and 100 ng/mL is 250 nmol/L. Indian labs usually report in ng/mL, while British and European labs and many research papers use nmol/L. Check the unit on your report before comparing it with anything you read online, because a result of 50 means something very different in each unit.

Why do different guidelines give different cut-offs?

Because they were answering different questions. In 2011 the US Institute of Medicine concluded that 20 ng/mL (50 nmol/L) covers the bone-health needs of nearly everyone. The same year the Endocrine Society, writing about patients at risk of deficiency, set 30 ng/mL as its threshold for sufficiency and called 20 to 29 insufficient. The UK goes lower still, treating 25 nmol/L (10 ng/mL) as the level below which bone health is at risk. Many Indian laboratories adopted the Endocrine Society's higher figure, so by the Institute of Medicine's standard, plenty of people labelled insufficient are already adequate.

What did the Endocrine Society change in 2024?

Its 2024 guideline stepped back from routine testing and from target numbers. It advises against routine vitamin D testing in healthy adults under 75, and it did not endorse any specific blood level as the goal, because trials had not shown that pushing levels above a particular threshold improved health outcomes. Instead it suggests vitamin D for certain groups, such as children, adults over 75, pregnant women and people with high-risk prediabetes, without testing first. Many Indian lab reports still print the 2011 bands.

What does a result below 20 ng/mL mean?

By almost every guideline, a level below 20 ng/mL is low, and below about 12 ng/mL is clearly deficient, with a real risk to bone health. In adults, prolonged deficiency can cause osteomalacia, a softening of the bones with aches and muscle weakness; in children it causes rickets. A low number is a reason to see your doctor, who will weigh symptoms, bone health, diet, medicines and other results before deciding on treatment. The dose, form and duration are medical decisions, and high-dose sachets taken repeatedly without supervision are a recognised route to toxicity.

What to do with a number below 20

  1. Check the unit first

    A result of 50 sits inside the sufficient band in ng/mL and at the deficiency line in nmol/L, so read the unit printed beside the number before anything else.

  2. Take it to your doctor

    Symptoms, bone health, diet, medicines and the other results all feed the decision, and below about 12 ng/mL is clearly deficient.

  3. Let the dose be prescribed

    Form, dose and duration are medical decisions. Repeated high-dose sachets bought without supervision are a recognised route to toxicity.

  4. Give it time before repeating

    25(OH)D takes several weeks to settle at a new level, so a repeat test is usually not worth doing before about three months.

  5. Use the same laboratory

    Assays differ enough that switching laboratories can move the number on its own, which hides the trend you are trying to follow.

Is a result of 20 to 29 ng/mL a problem?

It depends on the guideline and on you. By the 2011 Endocrine Society bands it is insufficient; by the Institute of Medicine's view it is adequate for bone health in most people. For a healthy adult without symptoms, it is often not a cause for alarm. For someone with osteoporosis, a history of fractures, a malabsorption condition or kidney disease, or someone on medicines that affect vitamin D, the doctor may choose to treat it. The same number can call for different decisions in different people.

Can vitamin D levels be too high?

Yes. Levels above about 100 ng/mL are generally treated as more than necessary, and toxicity is usually seen well above that, often beyond 150 ng/mL (375 nmol/L). The danger is high blood calcium, which can cause nausea, vomiting, thirst, confusion, kidney stones and kidney damage. Sunlight cannot cause it, because the skin limits its own production; toxicity comes from excessive supplements or repeated injections. A high result while taking vitamin D needs prompt review by whoever prescribed it.

Why does my vitamin D level vary between tests?

Because the level genuinely changes, and because tests differ. 25(OH)D rises after months of sun and falls after a winter or monsoon indoors, so a report from March can differ from one taken in September. Assays vary too: laboratories use different immunoassay machines or the more precise LC-MS/MS method, and results on the same sample can differ noticeably. International standardisation efforts have narrowed the gap without closing it. To track a trend, repeat the test at the same laboratory rather than switching between them.

What other tests might your doctor order with vitamin D?

Serum calcium, phosphate, alkaline phosphatase and parathyroid hormone (PTH) are the usual companions. Low vitamin D with raised PTH suggests the body is working hard to keep blood calcium normal, a pattern called secondary hyperparathyroidism. Raised alkaline phosphatase can point to bone softening. Kidney function tests matter because the kidneys activate vitamin D. Vegetarians are often tested for B12 at the same visit, for reasons the post on vitamin B12 for vegetarians explains. A bone density scan is sometimes added for people at risk of osteoporosis.

Do you need to fast before a vitamin D test?

No, fasting is not needed for 25-hydroxyvitamin D itself, and the test can be done at any time of day. If other tests are taken from the same sample, such as fasting glucose or a lipid profile, follow the instructions for those. Tell the laboratory or doctor if you take vitamin D or calcium supplements, or have recently had a vitamin D injection, because that changes how the result is interpreted.

How much does a vitamin D test cost in India?

The price varies widely by city, by laboratory and by whether the test is part of a health package, so any single figure would mislead. Standalone tests at large chains, hospital labs and small diagnostic centres are priced differently, and home collection may add a charge. Ask whether the lab is NABL-accredited and which unit it reports in. Given the 2024 guidance, it is also worth asking your doctor whether you need the test at all.

When should you repeat a vitamin D test?

Usually not before about three months after starting treatment, because 25(OH)D takes several weeks to settle at a new level. Your doctor will set the timing according to the dose and the reason for treatment. Testing every few weeks tells you little, and repeated testing in healthy people without symptoms is exactly what the newer guidance discourages. The honest summary

Your vitamin D report measures 25-hydroxyvitamin D. Below 20 ng/mL is low by almost any standard, 20 to 29 is where experts disagree, 30 to 100 is what many Indian labs call sufficient, and very high values carry real risk. Multiply ng/mL by 2.5 for nmol/L. The number is a starting point for a conversation with your doctor, not a prescription in itself.
