Sweet shops now keep a separate counter marked sugar free, and it is busiest in the fortnight before Diwali. What sits behind that glass ranges from a genuinely useful option to an ordinary sweet with a reassuring sign.

Sugar-Free Sweets for Diabetics: Is Mithai Labelled Sugar Free Actually Safe?

Ask a sweet shop owner who buys from the sugar-free counter and the answer rarely changes: families shopping for a parent with diabetes, usually in the weeks before Bijoya and Diwali. The counter exists because people want to include someone at the festive table without harming them, which is a kind instinct worth respecting.

The difficulty is that sugar free on a mithai tray covers very different products. Some cut the glucose load meaningfully, and some change almost nothing apart from the price. Knowing which is which, and how to test a sweet on your own meter, is more useful than a blanket yes or no.

Are sugar-free sweets safe for people with diabetes?

They can fit into a festive day for many people with type 2 diabetes, but they are not a free pass, and some differ very little from ordinary mithai. Sugar is only one ingredient. Khoya, chhena, milk powder, nuts, besan, suji and dried fruit remain, and several of these raise blood glucose by themselves. The ICMR-INDIAB study, which tested more than 113,000 adults across India, estimated diabetes prevalence at 11.4 per cent and prediabetes at 15.3 per cent, so a sizeable share of any family gathering is affected. Treat a sugar-free sweet as a somewhat better version of a sweet, eaten in the same small amounts.

What does "sugar free" on mithai legally mean in India?

Under FSSAI's claims rules, a food may be called sugar free only if it contains no more than 0.5 g of sugars per 100 g. The related claim, no added sugar, means no sugars were added during making, although natural sugars can still be present in quantity. Packaged foods containing artificial sweeteners must name the sweetener and state that the product is not recommended for children, and foods containing sorbitol must warn that it may have a laxative effect. Loose sweets on an open tray seldom come with any of this, so a handwritten card reading sugar free is not the same thing as a checked claim on a sealed, labelled box.

What replaces the sugar in sugar-free mithai?

Usually two things together: an intense sweetener for taste and a bulking ingredient for texture. Sugar does more than sweeten a barfi or a ladoo. It holds moisture, sets the fudge and gives body, so taking it out leaves a gap. Intense sweeteners FSSAI permits in Indian sweets include sucralose, aspartame, acesulfame potassium and saccharin, each within a maximum level, and steviol glycosides are also used. Bulk tends to come from polyols such as maltitol, sorbitol or isomalt, or from extra milk powder, nuts or dates. Whether each sweetener is safe in itself is answered in are sugar-free sweeteners safe; the concern here is the whole sweet.

Do sugar-free sweets raise blood sugar?

Most do, though usually less than the ordinary version. Intense sweeteners have almost no direct effect on glucose. Polyols differ from one another: a review of the glycaemic evidence gave erythritol a glycaemic index of 0, sorbitol and isomalt 9, and maltitol 35, against 65 for table sugar, and maltitol is among the commonest bulking agents in commercial sugar-free products. Then there is the rest of the recipe. Besan, suji and flour are starch, dates and figs are sugar in another form, and khoya carries lactose. A sugar-free besan ladoo can still push glucose up noticeably, while a sugar-free sandesh made from chhena with a little stevia may barely move it.

Why can a khoya barfi never be truly sugar free?

Because the sugar in milk comes with the milk. Khoya is milk simmered until most of its water has gone, and roughly a fifth to a quarter of its weight is lactose, the natural milk sugar. Lactose counts as a sugar on a nutrition panel, so a barfi made mainly from khoya cannot get down to 0.5 g of sugars per 100 g, whatever sweetener replaced the added sugar. What such a sweet can honestly claim is no added sugar. Lactose raises glucose less sharply than table sugar, but it is not nothing. Chhena, the fresh cheese behind sandesh and rasgulla, holds less lactose because much of it drains away with the whey.

Are sugar-free sweets lower in calories?

Only a little, in most cases. Sugar gives 4 kcal per gram, polyols roughly half that or slightly more, and intense sweeteners almost none, but sugar was never the largest source of energy in a rich sweet. The khoya, ghee, nuts and besan stay as they were. Replacing the sugar in a 30 g milk barfi with maltitol and sucralose might save perhaps 10 to 15 kcal, leaving most of the 100 to 140 kcal an ordinary piece of that size carries. For the many people with diabetes who are also trying to lose weight, that gap matters. The same arithmetic for a nut-based sweet is worked through in kaju katli calories.

Are date and anjeer sweets better for diabetics?

No, although they are often sold as though they were. Date and fig rolls, khajur barfi and anjeer ladoo carry labels such as no added sugar or naturally sweetened, which is accurate, yet dried dates are around two thirds sugar by weight. A 25 g piece of a date and nut roll can hold something like 12 to 16 g of sugar, more than many ordinary sweets of the same size. The fibre in dates slows absorption slightly, not nearly enough to cancel that load. Plain dates in small numbers can fit a diabetic meal plan, as can diabetics eat dates explains, but a box of date sweets is not a sugar-free box.

What about sweets made with jaggery, honey or coconut sugar?

These are sugar with a better reputation. Jaggery and nolen gur carry traces of minerals and a wonderful flavour, and honey and coconut sugar have loyal followers, but all are made mostly of sucrose, glucose and fructose, and all raise blood glucose in much the way white sugar does. A sandesh sweetened with nolen gur is a seasonal pleasure and a real part of the Bengali winter, not a diabetic sweet. How these sweeteners compare in detail is set out in jaggery, honey or sugar. A jaggery sweet described as sugar free would not meet FSSAI's definition, whatever the sign on the counter says.

Do sweeteners in mithai affect insulin or gut bacteria?

The evidence is mixed, and for occasional festive use it is not a strong reason for alarm. A 2022 randomised trial gave 120 healthy adults sachets of saccharin, sucralose, aspartame or stevia for two weeks, below acceptable daily intakes; saccharin and sucralose worsened glucose tolerance as groups, and responses varied from person to person in step with their gut bacteria. A 2019 BMJ systematic review, by contrast, found no convincing evidence of either harm or clear benefit in generally healthy people, with low certainty throughout. Neither involved people with diabetes eating mithai, so the fair reading is uncertainty rather than danger.

Can sugar-free sweets cause stomach upset?

Yes, and it is the problem people most often notice. Polyols are only partly absorbed in the small intestine, so the remainder draws water into the gut and is fermented by bacteria in the colon, which can cause wind, bloating and loose stools. That is the reason for the laxative warning on products containing sorbitol. Tolerance varies a great deal: one sugar-free piece may cause no trouble, while four pieces eaten over an evening of visiting leave the same person uncomfortable. Older adults, people with irritable bowel syndrome, and anyone already troubled by loose stools on metformin may notice the effect sooner than others.

Is erythritol in sugar-free sweets a concern?

It is an open question rather than a settled risk. Erythritol is a polyol chosen for some sugar-free products because it barely raises glucose. A 2023 study found that higher blood levels of erythritol were associated with more heart attacks and strokes over three years among people already under assessment for heart disease, and that eating erythritol kept blood levels raised for more than two days in a small group of eight volunteers. Associations cannot prove cause, and the body makes some erythritol of its own. Until better trials report, people with heart disease have a reasonable case for not eating erythritol-sweetened products in quantity.

Which sugar-free sweets are the better choice?

Those built on chhena, milk or nuts with little starch, sweetened with stevia or sucralose, and cut small. A sugar-free sandesh, a thin piece of sugar-free kaju katli or a baked chhena sweet generally affects glucose less than sugar-free besan ladoo, suji halwa, gulab jamun or motichoor, which stay starchy and are often fried. With packaged sweets, read the ingredients: if dates, maltodextrin, flour or glucose syrup appear near the top, the sugar-free claim is doing less work than it implies. Where you can, buy from a shop able to tell you which sweetener it used, and prefer a sealed box with a nutrition panel to an unlabelled tray.

How many sugar-free sweets can a diabetic eat?

One small piece on a festive day, eaten with a meal, is a sensible starting point for most people with well-controlled type 2 diabetes. That matches the portion advice for ordinary sweets, because the carbohydrate in the base ingredients has not gone anywhere. A consensus report on nutrition therapy from the American Diabetes Association notes that sweeteners reduce sugar and calorie intake only when people do not make up the difference elsewhere, and a sugar-free label tends to invite exactly that second or third piece. Eating the sweet at the end of lunch, with slightly less rice or roti, keeps the meal's total carbohydrate steady.

How to test a sugar-free sweet on your own meter

Your own glucose readings give a more reliable answer than any label. On a quiet day, check your blood glucose just before a meal, eat that meal with one piece of the sweet you plan to buy, and check again two hours after the first bite. On another day, repeat the same meal without the sweet. The difference between the two readings shows what that particular sweet does to you. Anyone using a continuous glucose monitor sees the whole curve, which is more informative still. Run the test in the week before the festival rather than on Diwali itself, when late meals and a broken routine make any single reading hard to read.

Testing a sweet before the festival

A simple paired test shows what one sweet does to your glucose, which no label can tell you.

  1. Pick a quiet day in the week before

    Choose an ordinary day with your usual medicines, activity and meal times, well before the festive routine starts.

  2. Check before the meal

    Take a finger-prick reading just before you start eating, and note what the meal contains.

  3. Eat the meal with one piece

    Have the sweet you plan to buy at the end of that meal, keeping the rest of the plate as usual.

  4. Check again after two hours

    Time the second reading from the first bite of the meal, not from the sweet.

  5. Repeat without the sweet

    On another day eat the same meal with no sweet and take the same two readings. The difference is the sweet's effect.

  6. Take the numbers to your doctor

    If the rise is large, or readings are already high, discuss the results before the festival rather than adjusting anything yourself.

Who should be careful with sugar-free mithai?

Several groups need more than a general rule. Anyone on insulin or sulphonylurea tablets must never use a sugar-free sweet to treat a low, because it will not raise glucose fast enough; a hypo needs the glucose or ordinary sugar their doctor has advised, taken at once. People with kidney disease often have limits on protein, potassium or phosphorus that rich milk and nut sweets can breach, and should ask their nephrologist first. FSSAI's labelling marks artificially sweetened products as not recommended for children. Women with gestational diabetes should agree any sweets with their obstetric team, who know the readings they are aiming for.

Explaining to a family why a sugar-free besan ladoo still lifts their father's glucose while a small sandesh barely does takes food chemistry, carbohydrate awareness and an understanding of diabetes medicines, all delivered in plain words. The honest verdict for the festive season

Sugar-free sweets are a modest improvement, not a solution. They remove added sugar but keep the milk solids, nuts, flour or dates that carry most of the calories and much of the carbohydrate, and their polyols can upset the stomach. Choose chhena or nut-based pieces over starchy or fried ones, read the label or ask the shop, eat one small piece with a meal, and test its effect on your own meter before the festival. Higher circulating erythritol was associated with more major cardiovascular events in patients undergoing cardiac assessment, and erythritol ingestion raised blood levels for days in eight volunteers. The American Diabetes Association consensus found no single ideal eating pattern and noted that non-nutritive sweeteners may reduce calorie and carbohydrate intake only if people do not compensate with other foods.
