Protein and creatine share a gym shelf and often a sentence, which leads people to assume they do the same job. They do not, and the order in which you add them matters.

Protein or Creatine: Which Should You Take First?

Walk into any gym and someone will tell you to take whey, someone else will swear by creatine, and a third person will say creatine is a steroid that wrecks kidneys. For a beginner on a limited budget, the practical question is simple: if you can only start with one, which should it be?

The answer depends less on the products than on what you already eat and how you train. This article explains what each one does, which comes first for most people, how creatine is taken, and the cautions that matter, particularly around kidneys and blood tests.

What is the difference between protein and creatine?

They are different kinds of substance with different jobs. Protein is a macronutrient found in dal, milk, eggs, meat and soy, and its amino acids are the raw material for building and repairing muscle, enzymes and much else. Creatine is a small compound the liver and kidneys make from three amino acids, arginine, glycine and methionine, and about 95 per cent of the body's store sits in skeletal muscle. There it helps regenerate ATP, the immediate energy currency, during short bursts of hard effort such as a heavy set or a sprint.

What does creatine actually do?

It lets muscles do slightly more high-intensity work before tiring. With fuller creatine stores you can often squeeze out an extra repetition or two on a heavy set, or recover a little faster between sprints. Over weeks of training, that extra work adds up to greater gains in strength and lean mass than training alone produces. The International Society of Sports Nutrition's 2017 position stand describes creatine monohydrate as the most effective nutritional supplement available for increasing high-intensity exercise capacity and lean mass. It does little for steady endurance work such as a long, easy run.

Should you take protein or creatine first?

Protein, and preferably from food. Creatine only amplifies training; it cannot build muscle if the diet does not supply enough protein and energy to grow. A typical vegetarian day of roti, rice, a katori of dal and vegetables often lands well under the 1.6 g per kg a day that a 2018 meta-analysis identified as roughly where extra protein stopped adding muscle gains. Deciding what to buy first

The order is not a matter of taste. One of these has nothing to work with until the other is in place.

  1. Work out your protein target

    The calculator gives a figure per kilogram of body weight. A vegetarian day of roti, rice, a katori of dal and vegetables often lands well below it.

  2. Fill the gap with food

    Dal, curd, paneer, eggs and soy come first, with a protein supplement only where meals genuinely fall short.

  3. Look at how you train

    Creatine pays off for people lifting or sprinting hard at least three times a week. It offers little to occasional training, to walking for fitness, or to someone mainly trying to lose weight.

  4. Then add creatine, if it applies

    Three to five grams a day of plain monohydrate, taken every day, is the whole protocol. Loading first is optional, and it upsets more stomachs than it helps.

  5. Tell your doctor before a blood test

    Creatine lifts serum creatinine without harming the kidneys, so the lab report needs that context attached to it.

When is creatine worth adding?

Once your protein and calories are in order and you are training hard at least three times a week, with weights or a sport that demands repeated sprints, jumps or lifts. It offers little to someone who trains occasionally, walks for fitness, or is mainly trying to lose weight. It is also no shortcut past a poor training programme: gains come from progressive overload, and creatine simply lets you do a little more of it each session.

How much creatine should you take?

Most research uses 3 to 5 g a day of creatine monohydrate, the plain form behind nearly every trial; newer forms have not been shown to work better. Taken daily without loading, this fills muscle stores in about three to four weeks. A loading phase of about 20 g a day, split into four doses, for five to seven days fills them within a week, followed by 3 to 5 g a day to maintain them. Loading is optional and more likely to upset the stomach. Timing matters far less than taking it every day, so pick whatever time you will remember.

Is creatine a steroid?

No. Creatine is a naturally occurring compound, made in your body and found in meat and fish, and it has no hormonal action. Anabolic steroids are synthetic derivatives of testosterone. Creatine is not on the World Anti-Doping Agency's prohibited list and is used openly by athletes in almost every sport. The confusion comes partly from its place in gym culture and partly from products sold as creatine that contain undeclared substances. Buying plain creatine monohydrate from a seller that publishes batch testing avoids most of that risk.

Does creatine cause water weight and bloating?

It causes some water gain, commonly 1 to 2 kg during a loading week and less without loading. The water is drawn into muscle cells along with the creatine, not stored under the skin, so muscles tend to look fuller rather than softer. For athletes in weight-class sports this matters, and for someone watching the scale during fat loss it can be discouraging, so expect the jump and do not mistake it for fat. Stomach discomfort is more likely with large single doses, and splitting the dose usually settles it.

Does creatine affect kidney function tests?

It can change the result without harming the kidneys. Creatine breaks down to creatinine, the waste product doctors measure in blood to estimate kidney function, so regular creatine use can raise serum creatinine modestly. That can make the calculated eGFR look lower than it truly is. Tell your doctor you take creatine before any blood test, so they can read the result accordingly or use a different marker such as cystatin C. Studies in healthy adults on normal doses have not shown kidney damage, but the lab report still needs that context.

Is creatine safe for people with kidney disease?

Not without specialist approval. Anyone with kidney disease, a single kidney, diabetes affecting the kidneys, or a raised creatinine on past tests should talk to their nephrologist before starting creatine. The safety evidence comes largely from healthy people, and damaged kidneys have less margin for error. The same applies to anyone regularly taking medicines that strain the kidneys, including painkillers such as ibuprofen. Do vegetarians respond better to creatine?

Often, yes. Meat and fish supply roughly 1 g of creatine a day to people who eat them, so vegetarians rely on what their bodies make and tend to have lower muscle creatine stores. Studies comparing the two groups have found vegetarians usually show a larger rise in muscle creatine after supplementing, and sometimes larger performance gains. Creatine monohydrate is normally made synthetically rather than from animal tissue, so it generally suits vegetarians and vegans, although capsules may use gelatin. Can you take protein and creatine together?

Yes. They do not interfere with each other, and many people stir creatine into a post-workout protein shake simply because it is convenient. Some research suggests creatine is taken up slightly better alongside carbohydrate and protein, which a shake or a meal provides. There is no need for a combined product; buying the two separately lets you control the dose of each and usually costs less per serving.

Should teenagers take creatine?

Ask a doctor first, and in most cases wait. Teenagers gain strength quickly from well-coached training and adequate food alone, and the safety research on creatine in adolescents is much thinner than in adults. Paediatric sports medicine bodies have generally advised against performance-enhancing supplements, creatine included, for under-18s. A sixteen-year-old eating enough dal, milk, eggs and rice and training consistently has far more to gain from sleep and technique than from any tub.

Does creatine cause hair loss?

The evidence is weak. The worry traces to a single small 2009 study of college rugby players in which a loading phase raised levels of DHT, a hormone linked to male-pattern baldness; hair itself was not measured, and the finding has not been clearly replicated. Reviews of trials in athletes have also not found more cramping, dehydration or muscle injury among creatine users. If you are genetically prone to hair loss and uneasy, skipping creatine is a reasonable personal choice, but harm has not been shown.

How do coaches decide which supplements an athlete needs?

By starting with food, training and recovery, and adding supplements only where there is evidence and a clear gap. That means working out energy and protein needs, checking hydration, and then weighing the short list of supplements with strong support, creatine among them. The honest summary

Protein and creatine are not rivals. Protein is a daily nutrient everyone needs, best met through food and topped up with powder only when meals fall short. Creatine monohydrate is an optional, well-tested extra for people already training hard, at 3 to 5 g a day, with a kilogram or two of water weight and a blood-test quirk to mention to your doctor. Sort the protein first, then decide whether creatine is worth it.
